D/HH Connectors

The D/HH connectors will connect people in their community with other people and social and health resources.

As trusted community members, they advocate, mentor, educate, reach out and help their

community access the services and resources they need.

In addition, they help members of their community gain knowledge, information, and support.

If you would like to be a D/HH connector, please get in touch with Carrie Pogue at cpogue@alliesincaring.org
