Position Summary
The Community Navigation & Program Coordinator oversees community-based services and provides resource navigation and support to community members, program participants, and apprentices. This position serves as a central point of contact for individuals seeking assistance, connects them to appropriate resources, supports community education and outreach activities, and maintains accurate program documentation.
Key Responsibilities
Program Coordination:
Coordinate assigned community programs and services, including participant intake, scheduling, referrals, and follow-up activities.
Track participant contacts, referrals, services, attendance, follow-up, and outcomes.
Assist in collecting program data, demographics, success stories, and other information needed for reporting.
Identify recurring community needs, barriers, and service gaps, and communicate them to management
Outreach and Engagement:
Connect with individuals in their communities through home visits, community centers, or local events.
Conduct community outreach and represent the organization at events, meetings, and partner activities.
Build and maintain relationships with community organizations and referral partners.
Maintain an up-to-date directory of community resources.
Education and Support:
Provide culturally relevant health education, teach coping strategies, and guide clients in managing conditions such as depression, anxiety, or substance use disorders
Coordinate and promote educational workshops, resource sessions, office hours, and community events.
Resource Navigation:
Provide individualized navigation assistance and connect participants with resources such as housing, food assistance, healthcare, health insurance, employment, education, transportation, public benefits, and other community supports.
Advocate for clients ensuring individuals receive appropriate care and follow-up
Support WFD participants identifying barriers to education and employment and connecting them to resources that promote successful participation in training and employment.
Share information and resources on worker supports, benefits, employment rights, healthcare access, and other community needs.
Qualifications
Experience in community services, human services, social services, workforce development, community health, or a related field.
Strong communication, organization, problem-solving, and follow-up skills.
Ability to work effectively with individuals from diverse backgrounds.
Comfortable working both in an office and in community settings.
Ability to maintain accurate records and confidential information.
Proficiency with Microsoft Office and electronic recordkeeping systems.
Bilingual English/Spanish
Knowledge of community resources in Southern New Jersey preferred.
